The damn things Tool windows keep closing.
ARRRGGHHHH so annoying.

    Maybe your workspace got changed somehow. Try going back to default.

    Window->workspace->essentials

    or window workspace reset pallet locations depends which photoshop you have installed

    have you got 'SPACES' turned on in your OS X settings, my photoshop kept losing windows/tools etc with 'spaces' on... got so annoying I turned it off.

    Are you hitting the tab key? That is a feature to turn all floating pallets on and off.
