What do I do with the apple 1099 form

Ok so I received the tax form for 2011 and it's a photo copy and blank.how do I report the earnings when I do my taxes?

    Contact Apple developer support or not sure if itunes connect has different support but that would be listed in itunes connect if so. You know how much they paid you so you can always list the amount on your takes. I'd talk to a tax pro about it.

    yeah i was thinking of just printing out all the income and showing it to the tax person..

    1099 is something apple should be sending to you filled out if you made over $600

    They emailed me a while back wanting me to fill out a w-9 so they could send me a 1099 but I have not received anything else since.
