Binary Rejected .... Small and large app icons should be similar

hi
my app rejected by apple coz :
We found that the app icon displayed on the device does not sufficiently match the large icon displayed on the App Store
In your case, the app icon displayed on the device is blank
my app for ipad screen

    You didn't. Include an icon when publishing

    So you have to match them, the icon you upload to "manage your application" and the icon you upload to portfolio..

    yes i did not include icon image during the publishing i thought it was Optional
