Trivia quiz template

What is the best way to implement true/false questions to the trivia quiz template? I added a column to the "questions" table to store the format. I'm getting stuck after this.

    Is this a free template? Is so, can you post a link to it so we know what you're referencing? If it's a paid template, please contact the seller of the template for support.
