Localizations - How-to?

So how do you go about localizing your apps. Do you have to submit a new build and create a new app in manage your apps or can you submit a separate localized binary as an update to your existing app. And can you do the app name in that language for the localized version or will it display the same name as every other store?

    I'm only planning on doing the description. I have a friend who used to live in japan and says If I add some japanese phrases to the name,description and or icon they will eat this game up. So this would be only available in the japan store.
